For new Coaches/Managers/Officials a Balmain DFC Registration Form, a Football NSW Prohibited Employment Declaration Form (re Child Protection Act), a Canterbury DSFA Code of Ethics Form and a Football NSW Accident Support Program Insurance Form must be completed and submitted on one of the official registration days.

For Coaches/Managers/Officials registered in the previous year a Balmain DFC Registration Form compiled from details available in the database will be emailed for checking/correction, signing and return. In addition a Football NSW Prohibited Employment Declaration Form (re Child Protection Act), a Canterbury DSFA Code of Ethics Form and a Football NSW Accident Support Program Insurance Form will also be provided for completion and return.

Identification Documents

Appropriate identification must be provided.

Please ensure that you bring a photocopy of your ID documents to registration day.

Acceptable forms of identification for Coaches/Managers/Officials must contain a photograph and are :

• Current Drivers License
  (must be current/valid in year of registration. ie. if a Drivers License was used for the previous registration and that Drivers License expired before the new registration day then new ID must be supplied for new registration)
• Current/Valid Passport
  (must be current/valid in year of registration. ie. if a Passport was used for the previous registration and that Passport expired before the new registration day then new ID must be supplied for new registration)
• Current RTA 18+ Card
• Current Student Card (must include date of birth)

Photographs

Photographs for Coaches/Managers/Officials are current for five years. Some Coaches/Managers/Officials may need to attend for a new photograph. All new Coaches/Managers/Officials must be photographed.

Additional instructions to assist with the completion of the Registration Form are included.

A signed and dated Football NSW Accident Support Program Insurance Form tear-off MUST be submitted with each registration (see link below to this form and tear-off).
